The Access & Disability Resources team understands how your disability impacts you in a learning environment so we can determine what support you need to be successful in your classes.
If we understand your needs through conversation and the interactive process, documentation may not need to be provided.

When Documentation is Needed
If there is not enough information about how your condition affects your learning, we will request documentation.
If documentation is required:
- We will explain what to send and how to send it.
- You may sign a form if you would like us to request records for you.
- You may be approved for temporary accommodations while documentation is pending.
Temporary accommodations:
- Are valid for one term.
- Become permanent once documentation is received and reviewed.
- Will expire the following term if documentation is not submitted.
Access will make three attempts to obtain documentation on your behalf. After the third attempt, Access will notify you that the final request has been made. At that point, it becomes your responsibility to obtain and submit the documentation.
How to Obtain Documentation
Documentation may be requested from qualified providers or agencies, including:
- Health care providers.
- High school or college records.
- Mental health care providers.
- Veterans Administration.
- Vocational Rehabilitation.
You may choose to:
- Ask your provider to complete our Verification Form.
- Submit copies of medical records, evaluations or diagnostic reports directly to Access.
Many providers offer online patient portals where you can download your records. Check with your provider if you are not sure how to access them.

Where Documentation is Stored
Documentation is stored electronically by Access and is kept separate from your academic record. Access to this information is limited to authorized staff and handled according to confidentiality standards.
